Dear Barbara,
I had the same experience of questions asked that were not covered in the manuals when I wrote the exam for Teaching Adults. There were many questions that referred to words and theories that no where in any of the manuals did it provide insight into the definition of these words. TESOL told me that they were revamping the manuals to match the exam. I believe that the exam was written by doctorate professor of english language because there were many questions on the theories of language acquisition which are presently taught in our universities. A little unfair to exam on something we are not taught in the manuals.

See page 438 and 439 in your foundation manual and refer back to page 433 to 435.
So you need 16 questions to test each student for 6 different levels, Beginner X 2, Intermediate X 2 and Advanced X 2.
For example: First question goal (from page 433)=introduce themselves using verb, "to be" in present tense. Question could be "please tell me who you are?" If they can answer the question using, "I am...." they are at least a beginner I.
Draw a chart with three columns using grammar structure and tense point down the left side (from page 433-435). The middle column would be for the questions but leave blank for now and in right column write down what the correct answers would be using the proper grammar and tense for that block's level. Then figure out what the question should be.
